New Patient Registration

We welcome any new patient who live within our practice area: anywhere within E2, except E2 8**, as this is classed as Hackney not Tower Hamlets. Registration takes place at the practice every day after 11.00am. 
You will need to bring with you:

  • 1x Photographic ID (For example a Passport or Driving Licence)
  • 1x Proof of Address, dated within the last 3 months (For example a Bank Statement & Utility Bill).
  • Your NHS number.  This can be found on your NHS card, sent to you when you registered with your last GP.  If you no longer have this card, contact your previous GP or the Health Authority. We use your NHS number to obtain your previous medical records.

Every new patient receives a Health Check. Your registration takes one week to process - if you have an urgent medical problem which requires attention before then, you will need to go to the Walk In Centre at The Homerton Hospital. It can take a considerable time for us to receive your medical records, so it is important to answer all the questions regarding your health.

If you wish to pre-register complete this Pre-registration form. Click on the "Send" button to mail your form to us.  Alternatively you may print off a registration form and fill it out. Then visit the surgery with the documents listed above.
